We are seeking a compassionate, organized, and proactive Lead Care Coordinator & Client Experience Specialist to help ensure that every client, family, caregiver, and nurse experiences professional, responsive, and high-quality service.
This role is ideal for someone who enjoys building relationships, solving problems, supporting caregivers, coordinating services, and making a meaningful difference in people's lives.
Position Overview
The Lead Care Coordinator serves as a central point of communication between clients, families, caregivers, nurses, and agency leadership. This individual helps ensure smooth daily operations, caregiver success, client satisfaction, and timely coordination of care.
The ideal candidate is highly organized, detail-oriented, compassionate, and capable of managing multiple priorities while maintaining professionalism and excellent customer service.
ResponsibilitiesClient Experience & Relationship Management
- Serve as a primary point of contact for clients and family members.
- Conduct welcome calls and assist with new client onboarding.
- Coordinate caregiver introductions and smooth transitions of care.
- Maintain regular communication with clients and families.
- Conduct client satisfaction check-ins and address concerns promptly.
- Help ensure clients receive consistent, high-quality care and service.
Caregiver Coordination & Support
- Conduct caregiver orientations and onboarding.
- Train caregivers on agency expectations, documentation requirements, and client-specific care plans.
- Support caregivers with scheduling questions and service-related concerns.
- Assist with caregiver retention and engagement initiatives.
- Help ensure caregivers are properly matched with clients based on skills, availability, and compatibility.
Scheduling & Staffing Coordination
- Create and maintain caregiver schedules.
- Manage call-outs, schedule changes, and coverage needs.
- Coordinate replacement caregivers when necessary.
- Monitor staffing needs and proactively address potential coverage gaps.
- Maintain accurate scheduling records and communication logs.
RN & Care Plan Coordination
- Track client assessments and supervisory visits to ensure timely completion.
- Coordinate schedules with Registered Nurses for assessments and supervisory visits.
- Communicate with nursing staff regarding client updates and care plan recommendations.
- Update and maintain client care plans as directed.
- Monitor due dates and follow up on outstanding documentation and compliance requirements.
Administrative & Quality Assurance Support
- Maintain accurate client and caregiver records.
- Document communications, service updates, and operational notes.
- Assist with quality assurance initiatives and service follow-up.
- Support agency leadership in maintaining high standards of care and professionalism.
- Help identify opportunities to improve client and caregiver experiences.
Qualifications
- Experience in home care, healthcare, healthcare administration, staffing, customer service, case management, or care coordination preferred.
- Excellent communication and interpersonal skills.
- Strong organizational and multitasking abilities.
- Ability to remain calm and solution-focused in a fast-paced environment.
- Strong attention to detail and follow-through.
- Comfortable learning scheduling, documentation, and communication software.
- Professional, dependable, and compassionate attitude.
Compensation & Growth Opportunities
- Training Period: $17.00/hour
- Compensation After Training: $23.00/hour to $25.00/hour
