RESPONSIBILITIES:

• Coordination: Coordinate the preparation and collection of Minimum Data Set (MDS) assessments, care plans, and updates for all residents in conjunction with the IDT.
• Data Management: Review, analyze, and compile MDS data to ensure accuracy and compliance with federal and state regulations.
• Communication: Collaborate with the care team, physicians, and family members to ensure effective care planning and coordination.
• Quality Improvement: Participate in quality improvement initiatives to ensure the highest level of care for our patients.
