This position exercises independent judgment and discretion in assessing client needs, developing comprehensive care plans, and coordinating services. The Case Manager provides psychoeducation, crisis intervention, and harm reduction support, whilemaintainingcompliance with agency standards and regulatory requirements. By engaging with internal and external partners,maintainingaccuratedocumentation, andparticipatingin community outreach, the Case Manager strengthensequitableaccess to services and enhances client well-being.This position will support the Re-Entryprogram, which is committed to breaking the cycle of incarceration in Colorado.TheRe-Entry Services program supports individuals transitioning from prison back into the community with the resources they need to succeed. Through direct services, advocacy, and personalized support, we help build pathways to employment, housing, and stabilityoffering real opportunities for a second chance.
