Community Liaison / Patient Relations Manager
Serenity Hospice – Monroeville, PA (Serving Allegheny & Westmoreland Counties)
Serenity Hospice is growing, and we’re excited to expand our compassionate care into Western Pennsylvania. We are seeking a Community Liaison / Patient Relations Manager to help develop and strengthen relationships with referral partners and support the continued growth of our newest branch.
This is a unique opportunity to play a key role in building Serenity’s presence in the Monroeville and Greater Pittsburgh region while making a meaningful difference in the lives of patients and families.
What You’ll Do
Build and maintain strong relationships with hospitals, skilled nursing facilities, assisted living communities, physicians, and other referral partners
Represent Serenity Hospice through client visits, presentations, networking events, and community outreach
Identify opportunities to grow referrals and support admissions goals
Collaborate closely with clinical and leadership teams to ensure exceptional patient and family experience
Plan and participate in educational events and marketing initiatives
Work independently, manage your territory, and meet growth goals
What We’re Looking For
Creative, outgoing, and self-motivated professional with a passion for achieving results
Minimum 1 year of experience in hospice or home care marketing/liaison role required
Strong understanding of hospice philosophy and end-of-life care
Proven ability to work within an interdisciplinary healthcare environment
Excellent communication, presentation, and relationship-building skills
Valid driver’s license, reliable vehicle, and willingness to travel throughout the territory
