Duties
- Conduct thorough client assessments to determine medical, psychosocial, and discharge planning needs using EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems.
- Develop individualized treatment plans that incorporate protocols, behavioral health considerations, and managed care requirements.
- Oversee utilization review and management processes to ensure appropriate resource use while maintaining compliance with HIPAA regulations and medical terminology standards.
- Facilitate discharge planning by collaborating with social workers, physicians, nurses, and other healthcare providers to ensure smooth transitions from hospital to home or other care facilities.
- Support patient service initiatives by providing education on treatment plans, medication management, and post-discharge instructions tailored to pediatric or adult populations.
