Duties

  • Conduct thorough client assessments to determine medical, psychosocial, and discharge planning needs using EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems.
  • Develop individualized treatment plans that incorporate protocols, behavioral health considerations, and managed care requirements.
  • Oversee utilization review and management processes to ensure appropriate resource use while maintaining compliance with HIPAA regulations and medical terminology standards.
  • Facilitate discharge planning by collaborating with social workers, physicians, nurses, and other healthcare providers to ensure smooth transitions from hospital to home or other care facilities.
  • Support patient service initiatives by providing education on treatment plans, medication management, and post-discharge instructions tailored to pediatric or adult populations.
  • Skills
  • Familiarity with EMR/EHR systems used for documentation, case management, and discharge planning.
  • Excellent communication skills for effective collaboration with multidisciplinary teams including social workers and behavioral health specialists.
  • Ability to navigate managed care policies while advocating for patient needs within compliance standards such as HIPAA.

Pay: $18.00 - $20.00 per hour
