Community Care Liaison

Serengeti Care is hiring a full-time, exempt Community Care Liaison to grow community partnerships, connect clients with care services, and support a smooth intake experience.

Pay: $67,000–$69,000 per year
Schedule: Monday–Friday, with occasional after-hours meetings or assessments
Reports to: Branch Manager

Responsibilities

• Build referral relationships with hospitals, clinics, senior centers, long-term care facilities, physician offices, and community partners.
• Represent Serengeti Care at networking events, client visits, workshops, and lunch-and-learns.
• Identify new client opportunities through outreach, relationship-building, and follow-up.
• Conduct client intake assessments and facilitate service agreements.
• Coordinate with branch leadership and staffing teams to ensure a seamless start of care.
• Track referral activity, intake outcomes, outreach efforts, and performance metrics.
• Maintain HIPAA, DSHS, confidentiality, and company brand standards.

Qualifications

• 1 year of experience in home care, long-term care, healthcare, intake coordination, community outreach, or marketing.
• Strong relationship-building, communication, organization, and follow-up skills.
• Experience with lead generation, referral development, or client engagement preferred.
• Valid driver’s license, reliable transportation, and ability to travel locally.
• Comfortable working independently and managing multiple priorities.
• Proficiency with Microsoft Office applications.
