Role Description
This role is ideal for an RN looking for flexible, as-needed work conducting client assessments, care plans and supervisory visits every 60 days for non-medical home care clients.
Responsibilities
- Perform initial client assessments and reassessments every 60 day.
- Develop and manage individualized care plans
- Conduct monthly phone check-ins with clients
- Provide caregiver supervision and support
- Assist with caregiver hiring, training, and competency evaluations
- Lead caregiver orientation and ongoing development
- Conduct unannounced home visits to ensure quality of care
- Obtain and manage physician orders
- Make referrals for additional patients services as needed
- Monitor and update patient care plans based on changes in condition
- Participate in in staff evaluations and performance management
- Assist in building mentorship and training programs
- Contribute to marketing and growth strategy efforts
Qualifications
- Active, Valid Registered Nurse License for the state of Wisconsin
- Experience in personal care services, home care, hospice or similar setting
- Minimum 1 year of supervisory or administrative healthcare experience
- Proven skills in nursing practice, patient care, and creating effective care plans
- Ability to provide leadership, mentorship, and guidance to nursing staff
- Strong organizational and management skills, with experience in overseeing daily healthcare operations
- Knowledge of healthcare regulations, compliance standards, and best practices
- Excellent interpersonal, communication, and conflict-resolution skills
- Commitment to delivering compassionate, high-quality care while maintaining professionalism at all times
