A large Certified Home Health Agency (CHHA) is seeking a compassionate and dedicated Registered Nurse to provide exceptional care to patients in their homes throughout Westchester. This is a great opportunity to make a meaningful difference while enjoying work-life balance and professional growth.
Responsibilities
- Assess patients and families for nursing and health-related needs.
- Provide professional nursing care to patients requiring part-time or intermittent home care services.
- Develop, coordinate, and update individualized care plans in collaboration with healthcare team members.
- Educate and supervise patients, caregivers, LPNs, and home health aides on patient care techniques.
- Report changes in patient conditions to physicians and update care plans as needed.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Educate patients and families on disease management, self-care, and prevention strategies.
- Evaluate the effectiveness and outcomes of patient care.
- Coordinate with community professionals and agencies to ensure continuity of care.
- Maintain accurate, timely, and compliant documentation of all nursing services.
- Ensure confidentiality and compliance with state, federal, and agency regulations.
- Participate in professional development, staff orientation, and educational programs.
- Perform additional duties as assigned.
Qualifications
- Current NYS RN license and registration required.
- 1–2 years of experience in medical/surgical nursing or home care preferred.
- OASIS experience a plus.
- Strong assessment and clinical judgment skills.
- Ability to educate and communicate effectively with patients and families.
- Must have reliable transportation and valid driver’s license.
- Basic computer proficiency and electronic documentation skills.
- Professional liability insurance required.
