Registered Nurse (RN) – Adult Care Facility | Flushing, Queens
$15K Sign-On Bonus Up to $10K Annual Tuition Reimbursement (up to $40K)

A well-established home care organization in Flushing, Queens is seeking a Registered Nurse to provide care within an assisted living setting. This is an ideal opportunity for an RN with home care experience looking to transition out of field visits into a more structured, facility-based role.

Schedule: Monday–Friday, 7:30 AM – 4:00 PM, No Weekends
Salary: Up to $105,000 $15k sign on bonus

Position Overview:
The Adult Care Facility Nursing Case Manager is a key member of the clinical team, responsible for delivering skilled nursing care and coordinating services for patients residing in an assisted living environment. This role focuses on clinical oversight, care planning, and ensuring quality patient outcomes.

Qualifications:

• Active New York State RN license
• 1–2 years of clinical experience required
• Home care or CHHA experience preferred
• Strong clinical and assessment skills
• Proficiency with electronic documentation systems
