Integrating physical and behavioral health needs with a broad scope of social determinants of health needs, we provide a truly holistic, patient-centered plan of self-management support. Additional experience in Psychiatrics a strong plus.Minimum of Master’s Degree in Nursing.APRN license in good standing in Massachusetts w/additional licensure in Maine, New Hampshire, Rhode Island, Pennsylvania, North Carolina, Florida and Connecticut a plus.10 years of experience managing complex patients.Additional Experience Desired:Willingness to obtain licensure in all states/regions where Senscio has operations.Experience/interest in telehealth service delivery.Passion for providing Chronic Care Management.
Salary : $120,000 - $135,000
