Case Manager
Status: Full-Time
Compensation: $35.71 - $37.91 hourly
Location: Austin Street, Forest Hills, NY
Position Summary
Queens Community House is seeking a qualified Case Manager with a strong commitment to social services and a passion for working with older adults.
Case Management services play a vital role in assisting older adults with functional impairments by helping them access the services, benefits, and entitlements necessary to age safely at home while maintaining their quality of life. The Case Manager will build trusting relationships with clients and caregivers while engaging them in a collaborative, problem-solving process to address their unique needs and support their well-being.
The Case Manager will manage an average caseload of approximately 70 older adults residing in Community Districts 4 and 6, including Elmhurst, Corona, Forest Hills, and Rego Park. This role requires providing support that is professional, timely, culturally sensitive, and client-centered.
Primary Responsibilities
Client Intake & Assessment
- Screen new potential case management clients through the intake process.
- Assess clients’ presenting concerns, including:
- Personal and family adjustments
- Financial challenges
- Physical and mental impairments
- Provide clients with accurate information regarding services, benefits, and entitlements.
- Conduct in-home assessments and reassessments to determine eligibility for services and continuation of care.
Care Planning & Service Coordination
- Identify and collaborate with clients’ informal support systems to encourage ongoing assistance.
- Develop comprehensive care plans that are culturally sensitive and maximize community resources.
- Serve as a liaison with external agencies and service providers.
- Arrange services, advocate for clients, and intervene with agencies on behalf of clients when needed.
Case Documentation & Client Support
- Maintain complete, accurate, and timely case records.
- Assist clients with accessing Supplemental Services Funds, including identifying needs and coordinating purchases and delivery of approved items.
- Support outreach activities and community engagement efforts as needed.
Collaboration & Professional Development
- Collaborate with colleagues and maintain knowledge of all QCH programs and services to facilitate referrals for clients and families.
- Participate in NYC Aging and other internal or external trainings to complete required annual training hours.
- Participate in staff development activities, annual Staff Development Day, supervision, and regular staff meetings.
- Work collaboratively as an effective member of the Case Management and QCH staff team.
- Perform additional duties and special projects as assigned.
Qualifications
- Bachelor of Social Work (BSW) degree required.
- Minimum of 2 years of experience working with the gerontological population.
- Bilingual in Russian required.
- Demonstrated cultural competency and sensitivity to the diverse needs of older adults in the program’s catchment areas.
- Knowledge of communication needs related to visual and hearing impairments.
- Strong multitasking and organizational abilities with a high level of energy and initiative.
- Proficiency with various computer applications and software; knowledge of the STARS database preferred.
- Ability to manage sensitive and confidential information with professionalism and discretion.
- Strong analytical and critical-thinking skills with the ability to resolve complex client-related issues.
- Excellent interpersonal, communication, and customer-service skills.
- Demonstrated ability to build trust and maintain positive professional relationships.
- Proven ability to follow through on assignments with strong attention to detail.
