This job was posted by https://joblink.maine.gov : For more information, please see: https://joblink.maine.gov/jobs/1302734 **Care Coordinator** **Full-time/Remote Position** **We are currently seeking a Care Coordinator to join our team in the Cumberland County (Portland) service areas.** If you enjoy working with older people and those with disabilities, and working from a home office in Maine, then come join the SeniorsPlus team! **The Care Coordinator will** work for SeniorsPlus in the State of Maine to assist clients in making informed choices to live safely and independently in their communities. **Responsibilities Include:**
- Coordinating care adhering to a philosophy of person centered, strength based, proactive planning while using a collaborative partnership approach and follows his/her applicable practice and ethical standards
- Completing psychosocial assessments, risk assessment, goal development, action plans, and back-up plans
- Adhering to contract requirements and current state program regulations in accomplishment of daily work
- Upholding the values of client respect, choice, access, dignity, quality, prevention and early detection
- Working with providers to gather information about Medicare or services supported by third party
- Insurers
- Ensuring evaluation of the effectiveness of the plan of care, making recommendations for care plan modifications and identifying barriers
- Adhering to all appropriate protocol related to face-to-face visits
- Bachelors degree in health or social services required
- Minimum of one year in Social/Human/Community Services required
- Familiarity with the long-term care system a plus, strong computer and organizational skills required
- Valid Driver\'s License a Must
- Must receive favorable results from driver\'s license, reference, and criminal conviction background checks for any offer of employment. **Full Benefit Package/Pay:**
