As a SAGE Memory Care Professional, your primary objective will be to offer personalized person-centered activity-based care and support for individuals with memory loss, assisting with Activities of Daily Living (ADLs) and fostering a positive and meaningful client experience.
$20.00 - $25.00 per hour | Full-time Positions Available

Responsibilities:
- Companionship & Emotional Support: Provide meaningful companionship and engage clients in activities like reading, games, and other mentally stimulating tasks that align with person-centered, activity-based care.
- Personal Care Assistance: Help clients with ADLs including hygiene, dressing, mobility, incontinence care, and feeding.
- Household Support: Perform light housekeeping, assist with errands, and provide transportation as outlined in the care plan.
- Monitoring & Reporting: Document completed tasks and regularly monitor clients, reporting any changes in their physical condition, cognitive ability, or behavior to Care Coordinators.
Requirements/Qualifications:
- Minimum of 3 years of experience in Dementia care.
- Ability to lift a minimum of 35 lbs.
- Valid Driver's License, vehicle with current registration, and insurance.
- Reliable means of transportation to travel to and from work.
- Ability to pass a background check.
- Proof of eligibility to work in the U.S.
