Registered Nurse (RN) – Home Health
Rate: $40-50 /per hour (15-20 hours per week)
Location: Sussex County, NJ
Job Summary:
We are seeking a compassionate and detail-oriented Registered Nurse to join our Certified Home Health Aide (CHHA) team. The RN will oversee patient care plans, perform initial assessments, supervise CHHAs, and ensure compliance with clinical standards and regulatory requirements. This position involves visiting clients in their homes to deliver skilled nursing services and coordinate quality care.
Key Responsibilities:
- Conduct initial and ongoing patient assessments.
- Develop and update individualized care plans.
- Supervise and train Certified Home Health Aides (CHHAs).
- Administer medications, treatments, and wound care as prescribed.
- Monitor patient progress and communicate updates to physicians and care teams.
- Maintain accurate and timely documentation in accordance with state and agency guidelines.
- Ensure compliance with home health regulations and best practices.
Requirements:
- Valid RN license in the state of New Jersey.
- Minimum of 1–2 years of nursing experience, preferably in home health or community care.
- Excellent clinical, organizational, and communication skills.
- Reliable transportation for home visits.
Pay: $40.00 - $50.00 per hour
Expected hours: 15.0 – 20.0 per week
Work Location: In person
