Client Care Coordinator

Senior Helpers of Baltimore and Central Maryland

Lutherville-Timonium, Maryland | Full-time | On-site with local travel

$50,000–$62,000 annually, based on experience, plus quarterly performance bonuses and benefits

Great home care starts long before a caregiver walks through the front door. It starts with listening, understanding what matters most, and helping a family feel confident about the next step.

We’re hiring a Client Care Coordinator who brings empathy, initiative, and strong relationship-building together into one meaningful role.

You’ll guide families from their first inquiry and assessment through enrollment and a successful start of care. Then you’ll own those relationships—staying connected, addressing concerns, and helping services evolve as needs change.

This role combines compassionate client service, consultative sales, and care coordination. Your ability to build trust and follow through will directly shape the client experience and our agency’s growth.

WHY JOIN US?

💜 The first and only home care agency in Maryland to earn CHAP Age-Friendly Care at Home Certification

💜 A team committed to raising the standard in home care

💜 A culture that believes great care starts with great people

💜 Meaningful ownership of client relationships, supported by experienced operations and nursing leadership

Our approach brings together the Age-Friendly 4Ms framework—What Matters, Medication, Mentation, and Mobility—with LIFE Profile assessments, Senior Gems dementia care, and hands-on caregiver training in our Center of Excellence.

WHAT YOU’LL DO

Help families choose and begin care:

• Respond promptly to qualified inquiries and maintain consistent follow-up.

• Conduct non-clinical assessments, including Senior Helpers LIFE Profile assessments, in homes, facilities, and other approved settings.

• Understand client routines, preferences, support needs, family expectations, and urgency.

• Recommend appropriate services, schedules, and care hours.

• Explain service options, pricing, and agreements; address questions; and guide families toward informed enrollment decisions.

• Coordinate onboarding, caregiver introductions, and timely service starts.

Build relationships that last:

• Serve as the primary non-clinical relationship owner for an assigned portfolio of clients and families.

• Lead proactive calls, client visits, first-day and first-month follow-up, and satisfaction outreach.

• Complete assigned non-clinical reassessments and service-plan updates.

• Recognize concerns early, coordinate solutions, and follow through to resolution.

• Recommend service adjustments or additional support when changing needs and family goals warrant them.

• Reconnect with prospective and former clients when services may meet their needs.

Keep care coordinated:

• Partner with nursing, scheduling, caregivers, and business development to support responsive, consistent service.

• Coordinate service agreements, authorizations, payer-program documentation, and care transitions.

• Escalate clinical concerns, safety risks, and serious complaints to appropriate leadership.

• Maintain accurate client, assessment, service-plan, and pipeline records in WellSky.

• Support billing accuracy, quality assurance, and documentation compliance.

• Review conversion, satisfaction, retention, and service-utilization trends to recommend improvements.

WHAT YOU BRING

• One to three years of relevant experience preferred in home care, healthcare, senior services, admissions, care coordination, consultative sales, client services, or account management.

• Confidence assessing needs, explaining solutions, discussing pricing, and helping people make decisions.

• Strong communication, organization, problem-solving, and follow-through.

• The ability to balance empathy with accountability for client experience and business results.

• Sound judgment and comfort managing competing priorities.

• Proficiency with Microsoft Office and comfort learning healthcare or client-management systems; WellSky experience is preferred.

• A valid driver’s license, current automobile insurance, reliable transportation, and availability for local travel.

• A bachelor’s degree in a related field is preferred.

Experience working with older adults and families is strongly preferred. Clinical assessments and decisions remain with appropriately licensed personnel.

WHAT SUCCESS LOOKS LIKE

Families receive timely, thoughtful guidance. Qualified inquiries become appropriate service starts. Clients feel heard and supported. Concerns are resolved, records stay current, and services evolve responsibly with each client’s needs.

You’ll report to the Director of Operations, with goals and priorities reviewed during your first 30, 60, and 90 days.

COMPENSATION & WORK ENVIRONMENT

• Annual base salary of $50,000–$62,000, based on experience.

• Quarterly performance bonuses.

• Benefits and PTO

• Full-time, based at our office at 1313 York Road in Lutherville-Timonium.

• Regular local travel throughout Baltimore and Central Maryland for assessments and client visits.
