Case Manager – Lee County

Are you looking for a meaningful career where every day you have the opportunity to make a lasting impact on vulnerable members of the community? Senior Friendship Centers is seeking a compassionate, organized, and self-motivated Case Manager to join our team.

In this role, you'll work directly with older adults throughout the community, assessing their needs, developing individualized care plans, and connecting them with essential services that help them remain safe, healthy, and independent in their own homes. This position meets with seniors in their homes to complete assessments in person.  Other related work can be completed remotely from home.  Candidates must be live in Lee County Florida and be comfortable driving their personal vehicle (mileage reimbursement included).

If you’re ready to make an impact, we’d love to hear from you—apply today!
