Team Select Home Care is currently seeking a dedicated and compassionate Internal RN to join our growing family. As an integral part of our team, the Internal RN will play a crucial role in providing exceptional care to our patients. This position involves a dynamic blend of office and field responsibilities, offering a unique opportunity to engage with both our healthcare professionals and patients.
Monday-Friday 9a-5p
Hybrid role (In office, at home/visiting patients within territory)
Responsibilities:
Patient Care:
- Conduct initial evaluation visits and reassess patients' nursing needs regularly.
- Coordinate and collaborate with healthcare professionals to develop and implement individualized care plans.
- Monitor patient progress and adjust care plans as needed.
- Provide hands-on patient care when necessary.
Documentation:
- Prepare accurate and timely clinical and progress notes for each patient visit.
- Summarize care conferences promptly following agency policies and procedures.
- Maintain comprehensive and up-to-date patient records.
Communication:
- Collaborate with the Director of Clinical Services/Nursing Supervisor to ensure effective communication and coordination of care.
- Communicate with patients, families, and healthcare team members to provide updates and address concerns.
Field and Office Time Management:
- Effectively balance time spent in the office and the field.
- Attend regular team meetings and training sessions.
- Ensure compliance with agency policies and standards.
Qualifications:
- Trach and Vent expereince
- Active and unrestricted RN license in the state of Florida
- Strong communication and interpersonal skills
- Knowledge of home healthcare regulations and standards
- Willingness to be on call as needed
