✔️ Provide in-home or on-site care at private residences, senior living communities, and long-term care settings
✔️ Manage chronic conditions, preventive care, and mental health needs for an aging population
✔️ Collaborate with interdisciplinary teams to deliver whole-person, senior-focused care
✔️ Build trusted relationships through empathy, consistency, and patient education
✔️ Support care access and equity by removing barriers for vulnerable seniors
