Physical Therapist (PT) – Medicare Home Health (Lymphedema Certified)
Location: Melbourne, FL

Reports To: Director of Clinical Services / Clinical Manager
Position Summary
We are seeking a compassionate and experienced Physical Therapist (PT) with specialized training in lymphedema management to join our Medicare-certified home health team. This role focuses on delivering high-quality, patient-centered care in the home setting, helping patients improve mobility, reduce swelling, manage chronic conditions, and enhance overall quality of life.
Key Responsibilities

Perform comprehensive physical therapy evaluations in accordance with physician orders and Medicare guidelines

Develop and implement individualized care plans focused on functional mobility and lymphedema management

Monitor and document patient progress in EMR in compliance with Medicare and agency standards

Educate patients and caregivers on home exercise programs, self-management techniques, and prevention strategies

Collaborate with interdisciplinary team members including nursing, OT, ST, MSW, and physicians

Participate in OASIS assessments and contribute to care coordination

Ensure timely documentation and adherence to all regulatory requirements
Qualifications

Active Physical Therapist license in the state of practice

Graduate of an accredited Physical Therapy program (DPT or equivalent)

Certified Lymphedema Therapist (CLT)

1 year of home health experience preferred

Strong knowledge of Medicare Part A home health regulations and documentation requirements

Experience with OASIS documentation preferred

Current CPR certification

Valid driver’s license and reliable transportation
