Physical Therapist (PT)
PRN – Home Health
Overview:
We are seeking a dedicated and compassionate Physical Therapists (PT) to provide high-quality, patient-centered home health services on a PRN basis. In this role, you will help patients regain mobility, improve functional independence, and achieve their rehabilitation goals—all from the comfort of their homes.
This position offers flexible scheduling and the opportunity to work independently while being fully supported by our clinical leadership team.
Key Responsibilities (PT):
Perform comprehensive evaluations and develop individualized plans of care for home health patients.
Provide therapeutic interventions to improve strength, mobility, endurance, balance, and safety.
Update and adjust treatment plans based on patient progress and clinical findings.
Educate patients and caregivers on exercises, safety, and home therapy programs.
Collaborate with nurses, OTs, SLPs, CNAs, and other team members to ensure coordinated care.
Complete accurate, timely clinical documentation in compliance with Medicare and agency standards.
Qualifications:Qualifications

Active Florida PT license.

Home health experience preferred (not required).

Strong clinical assessment and therapeutic skills.

Ability to work independently in a field-based environment.

Reliable transportation, valid driver’s license, and current auto insurance.
