Per Diem Medical Social Worker (MSW)
Overview:
We are a certified home health agency seeking a compassionate and experienced Per Diem Medical Social Worker (MSW) to join our team. In this role, you will provide support, resource coordination, and counseling services to patients receiving care in the home setting.
Key Responsibilities:

Conduct in-home assessments to identify needs affecting patient care.

Develop and implement individualized care plans to address identified needs in collaboration with the interdisciplinary team.

Provide counseling and crisis intervention to patients and caregivers.

Assist patients and families in accessing community resources such as financial assistance, housing support, transportation, and long-term care planning.

Educate patients and families on topics such as advance directives, health system navigation, and caregiver support.

Collaborate with nursing, therapy, and physician teams to ensure coordinated care.

Accurately document visits and interventions in the electronic medical record (EMR) system in a timely manner.

Master’s Degree in Social Work (MSW) from an accredited program (required).

Licensed Social Worker (MSW or LCSW) in the state of practice, in accordance with state regulations for home health services.

Minimum of one (1) year of experience in a healthcare setting; home health or hospice experience preferred.

Knowledge of Medicare home health regulations and psychosocial care practices.

Excellent communication, time management, and problem-solving skills.

Ability to work independently while adhering to agency protocols and quality standards.

Proficiency in using electronic documentation systems (EMR experience preferred).

Reliable transportation, valid driver’s license, and current auto insurance.
