🏡 About the Role
As a Home Health Licensed Practical Nurse, you will provide skilled nursing services to patients in their homes under the supervision of a Registered Nurse and in accordance with physician orders and Medicare guidelines.

💼 Key Responsibilities

• Provide skilled nursing care in the home under RN supervision

• Perform patient assessments and report changes in condition to the supervising RN

• Administer medications and treatments as ordered

• Provide wound care, catheter care, and other skilled nursing services within scope

• Educate patients and caregivers on medications, disease processes, and safety

• Accurately complete clinical documentation in the EMR in a timely manner

• Collaborate with RNs, physicians, therapists, and interdisciplinary team members

• Follow all Medicare, CHAP, and agency policies and procedures

✅ Qualifications

• Active LPN license in the state of Florida (required)

• Home health or Medicare experience preferred

• Experience using Wellsky/Kinnser Preferred 

• Current CPR certification

• Valid driver’s license, reliable transportation, and active auto insurance

• Strong clinical and communication skills

• Ability to work independently while remaining connected to a care team
