Position Summary

Senior Care Solutions is seeking a compassionate, dependable, and patient-focused Home Health Certified Nursing Assistant (CNA) to join our growing clinical team. The CNA provides hands-on personal care services to patients in their homes under the supervision of a Registered Nurse and in accordance with the patient’s individualized plan of care.

This role is ideal for someone who thrives in an independent environment, values meaningful one-on-one patient interaction, and is passionate about delivering high-quality home health care.

Key Responsibilities

• Provide personal care services including bathing, grooming, dressing, toileting, and oral hygiene

• Assist with mobility, transfers, ambulation, and range of motion exercises

• Monitor and report changes in patient condition to supervising nurse

• Assist with light housekeeping related to patient care

• Maintain accurate, timely documentation of visits in EMR

• Follow infection control, safety, and HIPAA guidelines

Qualifications

• Active Florida CNA License in good standing

• Current CPR certification

• Valid driver’s license and reliable transportation

• Auto insurance (meeting state minimum requirements)

• Ability to pass Level 2 background screening (AHCA compliant)

• Previous home health experience preferred but not required

• Experience with Wellsky/Kinnser preferred but not required

Work Environment

• Field-based position in patient homes
