At Senior Care Solutions, our team of professionals supports our clients’ patients and families through every stage of the hospice journey. We are currently seeking an Interim Hospice Case Manager and Interim Director of Hospice (Registered Nurse) to provide exceptional short-term clinical and operational leadership for our clients.
Position Summary
The Interim Hospice Case Manager / Director of Hospice plays a critical role in ensuring both continuity of patient care and effective hospice program oversight. This position is responsible for managing a temporary caseload while also providing leadership to the interdisciplinary team, supporting regulatory compliance, and maintaining high standards of hospice care.
The ideal candidate is an experienced hospice nurse leader who can step in seamlessly to deliver compassionate, patient-centered care while guiding staff and supporting organizational goals.
Responsibilities
- Provide skilled nursing care and expert symptom management for hospice patients.
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Oversee and coordinate care delivery across the interdisciplinary team, including physicians, social workers, chaplains, and nursing staff.
- Provide leadership, guidance, and support to clinical team members to ensure quality care and adherence to hospice standards.
- Support patients and families through education, emotional support, and ongoing communication.
- Ensure compliance with all regulatory, accreditation, and agency policies and procedures.
- Monitor quality metrics, identify areas for improvement, and support performance initiatives.
- Maintain accurate and timely clinical documentation in accordance with regulatory requirements.
- Participate in and, when appropriate, lead interdisciplinary team meetings to ensure continuity and coordination of care.
- Assist with operational oversight, including workflow management and staff coordination during the interim period.
Qualifications
- Active RN license in the state of Minnesota.
- Minimum 2–3 years of hospice nursing experience required; leadership or supervisory experience preferred.
- Strong clinical assessment, organizational, and leadership skills.
- Demonstrated ability to manage both patient care and team oversight responsibilities.
- Excellent communication skills with a compassionate, family-centered approach.
- Ability to work independently while also leading and supporting a team.
- Reliable transportation and valid driver’s license.

Pay: $47.00 - $50.00 per hour
