Senior Care Solutions is seeking a dependable, organized, and customer-focused Clinical Team Support professional to support our growing Medicare Home Health team. This unique weekend position is ideal for someone who thrives in a fast-paced healthcare environment and enjoys coordinating patient care while supporting multiple operational functions.
Working a Saturday, Sunday, and Monday schedule, the Clinical Team Support team member plays a vital role in ensuring seamless weekend operations by coordinating clinician schedules, supporting on-call activities, and assisting the Intake Department as needed.
Position Summary
The Clinical Team Support position is responsible for coordinating patient visits, communicating with clinicians and referral sources, supporting weekend on-call operations, and providing intake assistance as needed. This role requires exceptional communication, organization, and problem-solving skills to ensure patients receive timely, high-quality care.
Responsibilities
- Coordinate and schedule skilled nursing, therapy, and ancillary patient visits.
- Communicate with field clinicians regarding schedules, visit changes, and patient care needs.
- Support weekend on-call operations by assisting with incoming calls and urgent scheduling requests.
- Assist the Intake Department as needed by processing incoming referrals, gathering required documentation, communicating with referral sources, and supporting timely admissions.
- Coordinate same-day and priority visits to ensure continuity of patient care.
- Collaborate with Clinical Managers, Intake Coordinators, and field staff to optimize scheduling efficiency.
- Maintain accurate scheduling and patient information within the electronic medical record (EMR).
- Communicate professionally with patients, caregivers, physicians, hospitals, and referral partners.
- Escalate urgent clinical or operational concerns to the appropriate leadership team.
- Perform additional scheduling, intake, and administrative duties as assigned.
- High school diploma or equivalent required.
- Previous scheduling, intake, medical office, healthcare coordination, or home health experience.
- Medicare Home Health experience strongly preferred.
- Excellent organizational, multitasking, and time management skills.
- Strong communication and customer service abilities.
- Ability to work efficiently in a fast-paced environment while managing multiple priorities.
- Proficiency with Microsoft Office and electronic medical record (EMR) systems.
- Experience with WellSky/Kinnser is a plus.
