About the Role:
Are you a compassionate and dependable RN looking for flexible work in home health care? We're hiring a Per Diem Registered Nurse to join our growing team and provide skilled, in-home nursing visits to patients across Indian River County.
This role is perfect for nurses who enjoy autonomy, one-on-one care, and schedule flexibility, all while making a meaningful impact in patients' lives.
Responsibilities:

Conduct Start of Care (SOC) assessments, Recertifications, Resumptions of Care (ROC), and routine visits in the patient’s home

Complete thorough nursing assessments and create personalized care plans

Administer treatments and medications according to physician orders—including wound care and injections

Provide health education to patients and caregivers on medications, chronic conditions, and home care procedures

Collaborate closely with physicians, PT/OT/ST, and other members of the care team

Ensure accurate and timely documentation using WellSky EMR

Maintain compliance with Medicare standards, state regulations, and agency protocols
Qualifications

Active RN license in the State of Florida

Minimum of 1 year of clinical nursing experience (home health)

Strong communication and critical thinking skills

Familiarity with Medicare documentation standards

Must have reliable transportation, a valid driver’s license, and auto insurance

CPR certification required
