Job Summary
Semcac Senior Services Department is looking for a part time Social Worker to work with seniors and caregivers in Freeborn County . Flexible hours, starting at 20-24 hours per week with more available working in our other programs. Duties
- Providing information on respite care options to prevent caregiver burnout.
- Linking caregivers to training, coaching, or support groups.
- Helping a caregiver identify in-home supports for the care receiver.
- Follow-up activities focused on the caregiver's ability to sustain their role.
- Providing options counseling to older adults about home-and-community-based services.
- Assisting a senior in understanding Medicare or long-term care resources.
- Connecting a senior to chore, homemaker, transportation, housing, or benefits programs.
- Follow-up activities focused on ensuring the senior's needs were addressed.
