Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and overall condition; promptly report any concerns to healthcare professionals
- Administer medications accurately following care plans and maintain detailed documentation in compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Support clients with light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment
- Use specialized equipment like Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance
- Provide companionship and emotional support to enhance clients’ mental well-being and reduce feelings of isolation
Qualifications
- Prior experience in home health, assisted living, nursing home, or group home settings preferred
- Knowledge of patient observation, medication administration, and vital signs monitoring required
- Certifications such as CPR, First Aid, HIPAA compliance training highly desirable
- Experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer’s and memory care), or hospice needs preferred
- Ability to perform heavy lifting safely and operate mobility aids effectively
- Strong communication skills for working collaboratively with clients, families, and healthcare teams
- Valid driver’s license and reliable transportation are a plus for client visits
