Job Summary
Become a Self-Direction Community Habilitation Staff and make a meaningful difference in the life of a high functioning individual with developmental disabilities. In this role, you will provide person-cenetered support that promotes independence,. Your energetic and compassionate approach will empower this individual to achieve their goals while you fostering a safe, respectful, friendly and trustworthy environment.
Duties
- Facititae individual tailored goals to his preferences, and priorities.
- Maintain accurate documentation of services provided, case notes and transperency.
- Ensure a person-centered approach is used in all aspects of service delivery.
- Willingness and required to transport individual to and from schools to watch bus dismissal, school bus garages, to set up school bus garage tours, and trandport to and from activities that are the preference and priority of the individual.
- Proff of daily mileage is required foir reiembursemewnt.
- Are with individual at all times.
- Ability to be patient, build trust and respect and ensure saftety of individial.
- Shift starts when you pick up the individual from his home, in Lyons, NY, and he is in your vehicle.
- Shift ends when you drop off the individual to his home and I am required to sign your timesheet daily.
- A lot of daily travel is involved and will be reimbursed monthly at $.70 a mile.
- Shifts are Mondays-Thursdays 12:30pm-6:00 pm and Fridays 12:30pm-7:30 pm.
Requirements
- Must have 1 year exsperience working with people with developmental disabilities or autism spectrum disorder.
- Knowlege of Self-Direcetion Services a plus.
- Knowlege of person-cenetered care.
- Valid driver’s license with reliable transportation.
- Background check and finger printing required.
- Effective communication skills to build trusting relationships with the client and co-employer.
- A lot of daily travel is involved, and it will be reimbursed monthly at $.70 per mile.
Pay: $28.00 per hour
Expected hours: 28 – 30 per week
