We are seeking a Full-Time LPN for St. Joseph County and the surrounding areas and who has the heart and skills to work with home bound patients.
- Provide care wherever the patient calls home.
- Carry out the plan of care as directed by the patient's physician.
- Work with the physician for medication needs and changes to the plan of care.
- Ability to complete documentation within an EMR System. (WellSky)
- Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
- When appropriate, teach and counsel family members about care and process for disease/injuries.
- Attend orientation, meetings and CEUs,
Requirements
- Computer skills; must be able to work within EMR system.
- LPN's must have current LPN MI license in good standing.
- Reliable auto and current auto insurance.
- Current CPR.
- Can work well independently
- Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health
