Responsibilities
- Evaluate patient needs and assist in developing therapy plans of care.
- Provide treatment within legal scope, adhering to the plan of care and coordinating with team members.
- Monitor and report patient responses and condition changes to relevant healthcare professionals.
- Educate patients, families, and team members about care plans and progress.
- Complete evaluations, develop treatment plans, and document all patient interactions.
- Current Physical Therapy license in the practice state.
- CPR certification and valid driver’s license with vehicle insurance or access to transportation.
- Ability to work effectively in diverse home environments and with all patient demographics.
