- Deliver one-on-one physical therapy in private residences and senior communities.
- Perform evaluations, develop personalized care plans, and implement evidence-based treatments.
- Support patient mobility, independence, and wellness through therapy sessions.
- Document care accurately within the EMR system, adhering to regulatory standards.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Provide telehealth/virtual care to enhance access and continuity.
- Valid Physical Therapy license or eligibility in practice state(s).
- Degree from an accredited Physical Therapy program.
- Proficiency in basic computer use and EMR systems.
- Current CPR certification.
