Seeking licensed Physical Therapists to provide personalized in-home and senior living care, enhancing mobility and independence for older adults.
- Deliver one-on-one physical therapy sessions in private residences and senior communities.
- Perform evaluations, develop individualized care plans, and implement evidence-based treatments.
- Support patient wellness and functional independence through therapy.
- Document patient progress accurately within electronic medical records.
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Utilize telehealth and virtual care to extend access and flexibility.
- Valid physical therapy license or eligibility in practicing state.
- Degree from an accredited program.
- Proficiency in computer and EMR systems.
- Current CPR certification.
- Ability to exert medium effort, lift up to 50 lbs, and assist with patient transfers.
- Perform CPR and reposition patients as needed.
- Drive to patient locations and wear PPE when required.
