- Participates in care planning, case finding, and referrals for patients with complex social needs.
- Completes psychosocial assessments, develops and implements care plans, and documents interactions per standards.
- Supports patients and families with coping strategies, social support, and community resource referrals.
- Conducts consultations on abuse, neglect, advance care planning, and legal decision-making.
- Assists with discharge planning, ensuring appropriate community placement and post-discharge support.
- Contributes to quality improvement, performance audits, staff education, and departmental activities.
- Master’s degree in Social Work with a valid license (LCSW or equivalent).
- Minimum one year healthcare experience.
- Knowledge of medical terminology, psychosocial impacts of illness, community resources, and care processes.
- Strong interdisciplinary collaboration, communication, and customer service skills.
