PURPOSE OF THE POSITION: The Placement Coordinator (2) is responsible for responding to and assessing placement referrals to SCO’s Foster Care Program, residential treatment programs and facilities, day and residential schools programs. The Intake Coordinator collaborates with Intake, Homefinding, RJMCC and school staff to make the best possible match for youth referred to us by DOE or ACS’s Office of Placement Administration for residential school and foster care placement, respectively.

Position Requirements
- Review and assess placement referrals to determine if youth can be matched with a foster family in our family foster care program.
- Review and assess kinship placement referrals to determine if youth can be served by our family foster care program.
- Review and assess Department of Education residential school placement referrals to determine if youth can be served by SCO’s Tyree Learning Center and Madonna Heights School programs. Assist with assessment of residential placement referrals as needed.
- Review and assess residential treatment center foster care placement referrals to determine if youth can be served by our residential program. Assist with assessment of family foster care program placement referrals as needed.
- Request, review and assess educational and clinical assessments of children being referred. Request additional educational or clinical assessments if not available to aid in determining level of care needed, or if initial placement referral material is not clear.
- Respond timely to all placement referrals in the Connections Placement Module, referred via mail, phone and/or email.
- Enter and maintain placement referrals in agency referral database. Produce referral progress notes in agency referral database documenting resources explored, feedback on referrals from program partners, and requests for interviews or additional information needed to make a final determination for placement.
- Upload supporting documents to and maintain designated online folders used by program partners to review referral materials.
- Enter and maintain Kinship referrals in in agency referral and home certification databases. Accept, maintain, and assign FAD stages in Connections to Homefinding staff.
- Obtain case assignment for all accepted referrals and distribute draft and finalized intakes.
- Partner with family foster care Placement Coaches to identify and recommend foster home resources for referred youth not initially identified via a multi-facility search in the Connections Placement Module.
- Partner with residential treatment center staff to identify appropriate residential placement for referred youth.
- Partner with Tyree Learning Center and Madonna Heights School staff to identify and recommend appropriate school placement for referred youth.
- Process step-down and external replacement requests for residential treatment center residents. Assist with replacement requests between SCO’s other foster care programs and external placement referrals to other agencies.
- Maintain Kinship Log. Assist with maintenance and updating of other intake tracking logs/lists.
- Coverage of Intake Co-worker responsibilities, and completion of other responsibilities as assigned by Intake Supervisor and Program Director
- LMSW required, LCSW preferred
- Bilingual preferred
- Strong communication skills in writing and speaking
- Strong interpersonal and assessment skills
- Ability to engage professional colleagues of various levels within SCO and from
- our ACS and Voluntary Agency Partners.
