Job Summary: We are seeking a highly skilled and compassionate Telehealth RN Nurse Manager to join our dynamic healthcare team. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services. As a Telehealth RN Case Manager, you will work closely with patients, healthcare providers, and other stakeholders to ensure continuity of care, promote patient education, and facilitate positive health outcomes. This role requires strong clinical knowledge, excellent communication skills, and the ability to leverage technology to deliver exceptional patient care remotely.

Responsibilities

  • Lead, mentor, and manage a team of RN case managers to ensure high-quality patient care and performance standards.
  • Design, implement, and continuously improve clinical workflows and care management processes.
  • Define operational and clinical requirements in collaboration with product, technology, and leadership teams.
  • Conduct initial patient assessments and oversee development of individualized care plans.
  • Utilize telehealth platforms to monitor patient progress, ensure care quality, and support team efficiency.
  • Review, analyze, and interpret care management reports, dashboards, and key performance metrics to drive improvements.
  • Collaborate with cross-functional teams (product, sales, support, and clinical staff) to optimize care delivery models.
  • Engage directly with patients when required to address escalations, ensure satisfaction, and maintain quality of care.
  • Ensure accurate and timely documentation in electronic health records (EHR) systems.
  • Oversee patient education initiatives and ensure adherence to best practices in care management.
  • Coordinate referrals to community resources and support services as necessary.
  • Stay updated on healthcare trends, telehealth regulations, and best practices in case management and digital health.


Requirements

  • Current unrestricted RN license in Michigan
  • BLS Certification required
  • Minimum 5-7 years of clinical nursing experience, with leadership or case management experience preferred
  • Strong analytical skills with experience reviewing reports, dashboards, and performance metrics
  • Proven ability to design workflows, processes, and operational improvements
  • Tech-savvy with proficiency in telehealth platforms, digital tools, and EHR systems
  • Excellent communication and stakeholder management skills
  • Strong critical thinking, problem-solving, and decision-making abilities
  • Ability to lead teams while also working collaboratively across departments
  • Experience interacting with patients in both clinical and escalation scenarios
  • Compassionate and empathetic approach to patient care
  • Flexibility to adapt to evolving healthcare environments and technologies
  • Commitment to maintaining patient confidentiality and privacy
