Position Competencies:
• Ability to relate to individuals who have severe impairments in their ability to function in the community.
• Ability to respond quickly, effectively and with equanimity to member crises and life situations.
• Basic computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and negotiate through various screens or windows to complete daily chart documentation.
• Knowledge of Los Angeles County Department of Mental Health’s Rehab Option model of service provision and documentation requirements.
• Ability to work as a team member and to work with other agencies, community resources and client’s families.
Position Requirements:
• Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
• High School Diploma or Associates Degree in mental health or related field.
• Bachelor’s Degree in Social Work, Psychology or related field preferred.
• Must have at least four years of experience working in the field of mental health / social services with a HS Diploma.
• Must have at least two years of experience with an AA degree in the field of mental health / social services.
• Must have at least one years of experience with a BA degree in the field of mental health / social services. Must have valid California Driver's license and the availability of a car with adequate insurance.

Primary Duties:
• Provide targeted case management and mental health rehabilitation services to clients, linking them with all appropriate community services. Services are conducted in the field.
• Work as a team with clinician, psychiatrist and other inter/intra agency staff to provide consistent services.
• Conduct needs assessment with the client in the beginning treatment and on an ongoing basis when necessary to identify rehabilitation and linkage needs of the client. Collaborate with clinician and client to identify areas needed to be included in the treatment plan.
• Ensure clients are involved in establishing goals impacting their functional impairments. Involve clients in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible.
• Meet with all assigned clients based on frequency stated in the client’s treatment plan. Monitor client’s participation in services.
• Conduct mental health rehabilitation and recovery oriented services both individually and in groups as necessary. Provide mental health rehabilitation services, such as, counseling, money management, social skills training, and other independent living skills as needed by assigned clients.
• Collaborate with client and treatment team as necessary to assist clients in linkage to community resources including but not limited to: housing resources, obtaining benefits, obtaining basic necessities, health services, and linkage to transportation.
• Track client’s medication to ensure that they are medication compliant and collaborate with medication support team to ensure ongoing compliance and attendance to doctor’s appointments.
• Provide crisis intervention to clients at risk of hospitalization and /or incarceration in an effort to avert hospitalization and /or incarceration. Participate in safety planning with clients to ensure clients have knowledge and access to crisis intervention services after hours.
• Maintain contact with client’s family and/or significant other support persons as appropriate.
• Maintain productivity of 6 hours of billable service per day and submit progress notes in electronic health records within 48 hours of direct services. Documentation is to be completed and submitted accurately and in a timely manner. The services delivered are to be a documented in a manner consistent with established standards for the assigned program.
• Prepare for weekly clinical staff meetings by having current information on the status of all assigned clients
• Other duties as assigned.
