As a vital member of our team, you will assist the Director of Nursing and the RN Assessment Coordinator in ensuring that our documentation meets Federal, State, and Certification guidelines. Principal Responsibilities
- Assist the center in assuring adherence to Federal and State regulations and certification
- Actively participate in the regulatory or certification survey process and the correction of deficiencies
- Report trends from completed audits to the Quality Assurance Committee
- Assist in the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care
- Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the PointClickCare system
- Follow up with staff when necessary to assure compliance to standards of documentation
- Complete patient assessments, data collection, and interviews with staff as necessary to assure good standard of practice
- Facilitate accurate determination of the Assessment Reference Date that accurately reflects the patient's care needs
- Ensure timely submission of the MDSs to the State with proper follow-up on validation errors
- Maintain validation records from the submission process in a systematic and orderly fashion
- Graduate of an approved Registered Nurse/Licensed Vocational Nurse program and licensed in the state of practice required
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred
- Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement
- Through understanding of the Quality Indicator process
- Knowledge of OBRA regulations and Minimum Data Set
- Knowledge of the care planning process
