- Key responsibilities:
- Conduct client intakes, needs assessments, and develop individualized service plans.
- Deliver or coordinate services and referrals; maintain client contact and follow-up.
- Track outcomes, maintain accurate case notes and client databases (ensure confidentiality).
- Coordinate with partner agencies and internal teams to close service gaps.
- Participate in quality improvement, data collection for grant/reporting requirements.
