MISSION STATEMENT:

The mission of Sarah A. Reed Children’s Center is to improve the quality of life for children with personal, social, and emotional problems. Through teamwork, we provide a broad range of high quality, innovative, and effective mental health services to help children reach their potential and experience success in their families, schools, and communities.

CASELOAD:

Children 5 to 19 years of age. All essential functions are conducted within the framework of cognitive, physical, social, and emotional aspects of the developmental stage.

OVERALL PURPOSE OF JOB:

This is a professional position responsible for managing and coordinating a multitude of facets of a child’s treatment in the Partial Hospitalization Program. Works closely with the assigned therapist and clinical care staff to complete evidence-based treatment plans and ensure on-going quality of treatment.

ESSENTIAL FUNCTIONS OF JOB: (other duties may be assigned)

1. Coordinate all aspects of treatment for each client ensuring that the child/family’s needs are being met, and they have a sense of predictable future planning.

KEY MEASURES:

· Develop and maintain effective working relationships within the Partial Hospitalization Program and as well as with partners and providers outside the agency to ensure quality, seamless service for all children and families.

· Participate in classroom consultation, weekly staffing’s, and doctor meetings with clinical care staff, program supervisor, community provider agencies, schools, and family support groups

· Assist with referrals for other services, providing assistance as needed.

· Participate in interagency treatment meetings

· Coordinate discharge planning and organize necessary services; participate in school transition meetings

· Schedule and facilitate discharge planning, reauthorization, and transition of care meetings to ensure continued treatment and safe transition to other community resources.

· Complete and send notification of interagency meetings to families and referring agencies

· Attend court hearings as requested.

2. Develop and coordinate treatment plans using the SELF-model for each client.

KEY MEASURES:

· Develop initial treatment plan within 5 days of a client’s admission in collaboration with the therapist and clinical care staff and update every 20 days thereafter.

· Develop goals and objectives that directly relate to identified client needs based on SELF.

· Meet, review, and discuss treatment plans with all participants each time they are written and revised

· Seek feedback and have ongoing collaboration with direct care personnel and other treatment team members for progress updates

3. Establish and maintain positive, effective working relationships with assigned clients, their family, and colleagues. Model effective advocacy strategies.

KEY MEASURES:

· Create opportunities for families to participate in regular meetings to develop competence and an expectation of success

· Collaborate with individual therapists and clinical care staff as needed to review treatment goals and progress and ensure open communication is maintained and nurtured.

· Provide client and team members opportunities to express themselves or disagree with recommendations regarding readiness for transition into least restrictive level of care and/or continued treatment.

· Maintain on-going open communication with internal and external teams

· Participate in all meetings involving client treatment to ensure clients and families receive the most appropriate services.

· Be proactive regarding collaboration with providers, families and any assigned county support

· Create an environment of compassion and support by respecting the rights of the client, parents, and families.

4. Complete job-related reports, notes and documentation and maintain current client information.

KEY MEASURES:

· Complete treatment updates for all clients.

· Complete court summaries prior to court reviews.

· Update files as needed.

· Gather information for educational psychiatric reports

· Complete discharge instruction sheet, transfer form, and discharge summary.

5. Coordinate treatment with insurance provider.

KEY MEASURES:

· Complete plan of care for reauthorizations packets.

· Submit reauthorization packets to appropriate insurer.

· Participate in peer reviews as needed.

· Complete discharge paperwork within 24 hours.

· Assist families/referring agencies with grievance procedures.

OTHER DUTIES OF JOB:

1. Assist in planning, coordinating, and attending parent/caregiver outreach events.

2. Adhere to and promote a Sanctuary model of interaction with other departments using the 7 commitments and SELF model.

3. Provide support within the milieu as schedule permits

4. Adapt to ongoing programmatic needs and trends

5. Perform other related duties as assigned and required by immediate supervisor.

SUPERVISION RECEIVED:

Supervision is received by the Director of Clinical Services at PHP.

MENTAL/PHYSICAL REQUIREMENTS OF JOB:

This position typically requires:

Must be able to walk, sit, and stand throughout the workday as needed, ability to run occasionally, ability to talk, hear, read, and write, ability to lift up to 20 pounds occasionally, and ability to operate a vehicle.

WORK ENVIRONMENT:

This position typically requires:

Exposure to bloodborne pathogens, other communicable diseases/infections, and bodily fluids. Exposure to aggressive behavior.

QUALIFICATIONS:

A. Education/Training/License/Certification:

Minimum of bachelor’s degree in social services or related field.

B. Work Experience:

Minimum of two to four years of related experience is required.

C. Conditions of Employment:

Daytime work shifts, 5 per week. Some flexibility required

Obtain required clearances.

Complete required agency training.

Comply with all agency policies and procedures.

Valid Pennsylvania Drivers License

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

1. Knowledge of early childhood/childhood/adolescent development, behavior, and peer relationships.

2. Ability to read, analyze, and interpret professional journals and publications.

3. Ability to write reports, correspondence and procedural manuals.

4. Requires flexibility, leadership skills, and good judgment.

5. Excellent documentation and observation skills.

6. Excellent communication skills, both verbally and in writing.

7. Effective interpersonal skills and ability to establish and maintain effective working relationships with the children, their families, other staff, and other agencies.

8. Working knowledge of mathematical concepts and ability to apply concepts to practical situations.

9. Ability to define problems, collect data, establish facts, and draw valid conclusions.

10. Ability to conduct self in a positive manner to serve as role model for staff, families, and children.

11. Must be a mandated reporter of child abuse.

12. Must be able to work in a stressful environment.

13. Knowledge of and ability to use non-violent crisis intervention.

14. Follow all confidentiality and universal precaution guidelines.

15. Knowledge of computers.

Job Type: Full-time

Pay: $18.54 - $27.81 per hour
