Overall Purpose Of Job
This is a professional position responsible for managing and coordinating a multitude of facets of a child's treatment in the Partial Hospitalization Program. Works closely with the assigned therapist and clinical care staff to complete evidence-based treatment plans and ensure on-going quality of treatment.

Valid Pennsylvania Driver's License

ESSENTIAL FUNCTIONS OF JOB: (other duties may be assigned)
- Coordinate all aspects of treatment for each client ensuring that the child/family's needs are being met, and they have a sense of predictable future planning.
- Develop and coordinate treatment plans using the SELF-model for each client.
- Establish and maintain positive, effective working relationships with assigned clients, their family, and colleagues. Model effective advocacy strategies.
- Complete job-related reports, notes and documentation and maintain current client information.
- Coordinate treatment with insurance provider.
- Assist in planning, coordinating, and attending parent/caregiver outreach events.
- Adhere to and promote a Sanctuary model of interaction with other departments using the 7 commitments and SELF model.
- Provide support within the milieu as schedule permits
- Adapt to ongoing programmatic needs and trends
- Perform other related duties as assigned and required by immediate supervisor.
Qualifications
- Education/Training/License/Certification: Minimum of bachelor's degree in social services or related field.
- Work Experience: Minimum of two to four years of related experience is required.
