Key Responsibilities
- Assess residents' nutritional needs and develop individualized care plans
- Monitor nutritional status and modify interventions as needed
- Participate in interdisciplinary care plan meetings
- Ensure compliance with CMS, state, and facility nutrition standards
- Provide nutrition education to residents, families, and staff
- Document assessments and interventions accurately and timely
- Registered Dietitian credential (RD/RDN) required
- Current California license or eligibility for licensure
- Experience in skilled nursing or post‑acute care preferred
- Strong clinical, communication, and organizational skills
- Ability to work collaboratively in a team‑based environment
- Starting at $55/hour, depending on experience (DOE)
