About Us

The pretrial stage is a critical juncture in the larger criminal justice process; whether or not someone gets out of custody after arrest is a major determinant of the ultimate outcome of the case, as those who remain in jail are  more likely to face a conviction than those who are released. In addition, the presumption of innocence and constitutional protections against excessive bail are two of our most foundational legal principles. Despite these facts, and against the backdrop of mass incarceration nationwide, jail populations in San Francisco and across the country are disproportionately composed of pretrial individuals. 

We work to address this critical issue by providing alternatives to pretrial incarceration. By doing so, the San Francisco Pretrial Diversion Project (SF Pretrial) does its part to prevent unjust harm and unnecessary incarceration. Rather than languishing behind bars, newly arrested people can be released to our services to receive care, support, case management, and referrals to outside treatment providers. Our services are diverse and wide-ranging in recognition of the many needs that justice-involved people may have. Further, San Francisco has been the seat of new bail reform initiatives in California, which has afforded us the opportunity to continue to grow and build on our 45 years of experience overseeing pretrial services in the City and County.

Summary

SF Pretrial has been awarded a $7 million grant over two years to launch the Integrated Healthcare Initiative, a specialized healthcare coordination program serving justice-involved individuals with complex, intersecting needs. The initiative provides intensive support to clients experiencing homelessness, behavioral and physical health conditions, and chronic service disruption. Through partnerships with healthcare and housing providers, the program delivers coordinated care that addresses barriers to treatment engagement and improves health outcomes for our community's most vulnerable members.

The Pretrial Pathways Program Coordinator provides direct operational support for program implementation, focusing on client navigation, logistical coordination, and day-to-day communication across teams. This role ensures smooth program operations by managing schedules, maintaining information flow, supporting warm handoffs between providers, and providing hands-on assistance to clients navigating multiple service systems. Working under the direction of the Initiative Manager, the Coordinator executes established protocols, handles routine operational tasks, and serves as a responsive point of contact for immediate questions and logistical needs.

Essential Duties and Responsibilities

Direct Client Navigation and Support

  • Provide direct support to program participants navigating healthcare enrollment, housing placement, and treatment services

  • Conduct warm handoffs between SF Pretrial case managers, embedded partners, and external service providers

  • Follow up with clients who miss appointments or experience service disruptions to re-engage them in services

  • Maintain regular check-in communication with clients to identify barriers and troubleshoot access issues

  • Support crisis intervention by connecting clients to appropriate resources and escalating urgent needs

  • Assist clients with practical barriers (transportation, documentation, appointment scheduling)

  • Document all client interactions and service coordination activities in case management database

  • Track individual client progress through the service pathway and alert Initiative Manager to patterns or concerns

Operational Logistics and Scheduling

  • Manage day-to-day logistics for embedded partner presence at SF Pretrial offices

  • Coordinate schedules for partner staff on-site coverage to ensure consistent client access

  • Schedule and coordinate operational meetings between staff, case managers, and partners

  • Maintain shared calendars, contact lists, and communication channels for partner coordination

  • Serve as first point of contact for routine operational questions from staff and partners

  • Coordinate training logistics including room setup, materials preparation, and participant tracking

  • Manage program communications including staff updates, schedule changes, and resource distribution

  • Handle administrative tasks that support smooth program operations (file organization, materials ordering, meeting prep)

Data Collection and Documentation Support

  • Enter client screening results, referral information, and service activities into Open Justice database with accuracy and timeliness

  • Conduct routine data quality checks on own entries and flag missing or incomplete information

  • Support case managers with data entry questions and troubleshooting

  • Compile basic program activity reports (weekly referral counts, appointment tracking, enrollment updates)

  • Maintain organized filing systems for consent forms, assessments, and program documentation

  • Ensure all client documentation meets privacy and security standards

  • Assist Reporting and Compliance Manager with data collection requests and report preparation

Internal Communication and Information Flow

  • Serve as responsive point of contact for staff questions about referral processes, eligibility, and partner services

  • Distribute program updates, policy changes, and partner information to relevant staff

  • Maintain internal resource guides and FAQ documents based on common questions

  • Relay information between Initiative Manager and frontline staff to ensure timely communication

  • Document operational issues and solutions in shared knowledge base

  • Support onboarding of new staff by providing orientation to program protocols and systems

 

  • Perform other duties as directed, developed or assigned

Qualifications:

 

Qualification Requirements

  • Associate's or Bachelor's degree in social work, public health, human services, or related field, OR equivalent combination of education and experience

  • 1-2 years experience in social services, case management, healthcare navigation, or program support roles

  • Experience working with relevant populations (justice-involved individuals, people experiencing homelessness, or individuals with behavioral health challenges)

  • Strong organizational and time management skills with ability to handle multiple tasks

  • Excellent written and verbal communication skills

  • Proficiency with databases, Microsoft Office Suite, and Google Workspace

  • Ability to work both independently and collaboratively

  • Commitment to trauma-informed, culturally responsive service delivery

  • Comfort with ambiguity and changing priorities in a new program environment

Preferred Qualifications

  • Experience coordinating across multiple organizations or partner agencies

  • Experience with grant-funded programs and compliance requirements 

  • Knowledge of Medi-Cal enrollment processes and healthcare navigation systems

  • Familiarity with HIPAA regulations and data privacy requirements in healthcare settings

  • Bilingual English/Spanish 

  • Understanding of behavioral health and substance use treatment systems

Background Checks and/or Licensing Requirements

 Criminal Background check by the SF Sheriff’s Department for jail clearance and CLETS certification and reference checks.

Physical Demands 

  • Stand and walk or sit alternatively depending on specific needs of the day. Estimate 20% of time is spent on feet and 80% sitting at a desk.

  • Occasional need to perform the following physical activities: bending/stooping/squatting, climbing stairs, pushing, pulling, twisting, lifting and reaching above shoulders. 

  • Occasional need to perform standing and walking activities.

  • Constant need to perform the following physical activities: writing/typing, grasping/turning, finger dexterity.

  • Lifting/carrying up to 10 lbs. frequently and 25 lbs. occasionally. 

  • Vision requirements: constant need to complete forms, read reports, view computer screens. Frequent need to see small details. Frequent need to see things clearly beyond arm's reach.

  • Hearing requirements: constant need to communicate over telephone and in person.

The noise level in the work environment is usually moderate.
