Schedule: Monday–Friday, 8:00 a.m. – 2:00 p.m. (up to eight hours per day)No evenings or weekendsPosition Summary:We are seeking a compassionate and qualified Social Worker to join our Adult Day Health Care Center. The Social Worker will provide psychosocial assessments, care coordination, counseling, and support services to participants and their families, while working collaboratively with the interdisciplinary team to ensure comprehensive, participant-centered care in compliance with California Adult Day Health Care regulations.Key Responsibilities:Conduct psychosocial assessments and contribute to individualized care plansProvide individual and group counseling, support, and crisis intervention as neededAssist participants and families with resource referrals, benefits coordination, and community servicesParticipate in interdisciplinary team meetings and care planningMaintain accurate, timely, and compliant documentation in accordance with program and regulatory requirementsAdvocate for participants’ social, emotional, and psychosocial needsEnsure compliance with applicable state and federal regulations governing Adult Day Health Care programsQualifications:Master’s degree in Social Work, Counseling, or Psychology; ORBachelor’s degree in Social Work with a minimum of two (2) years of experience in a health care or social service settingExperience working with older adults or individuals with chronic conditions preferredStrong interpersonal, communication, and documentation skillsAbility to work effectively as part of an interdisciplinary teamWhat We Offer:Consistent weekday schedule (Monday–Friday)No evenings, weekends, or on-call requirementsSupportive, team-oriented work environment
