Arrowhead Regional Medical Center (ARMC) is recruiting for Community Health Workers (CHW)* - Extra Help for Street Medicine to provide outreach, engagement, and care coordination services to underserved populations, particularly individuals experiencing housing insecurity. Community Health Workers perform field-based work in community and non-traditional settings, including tent encampments, to connect patients with healthcare, social, and supportive services. Positions work collaboratively with healthcare providers and community partners to improve access to care, support continuity of services, and enhance health outcomes.

Duties may include:
  • Work primarily with designated target populations, including non-English-speaking individuals.
  • Develop and maintain effective working relationships with healthcare team members, community resource partners, patients, and families.
  • Perform intake procedures, including patient registration, insurance verification, eligibility review, and processing of patient records.
  • Provide customer service by answering phones, responding to inquiries, and taking messages.
  • Travel to various community locations, including tent encampments and other areas of need, to coordinate healthcare services.
  • Assist patients in navigating the healthcare system and accessing community resources, including housing, transportation, financial assistance, eligibility programs, and social or cultural services.
  • Facilitate patient referrals to the emergency department, specialty care, dental services, mental health services, substance use treatment, and other supportive resources.
  • Ensure mobile clinic supplies are ordered, inventoried, stocked, and replenished on a daily basis.
  • Perform other related duties as assigned.
Note: This is an Extra-Help position. Successful candidates shall be considered at-will throughout the term of their employment and will not attain status as a regular county employee.


ARROWHEAD REGIONAL MEDICAL CENTER
Located on a beautiful 70-acre campus in Colton, California (50 miles east of Los Angeles). Arrowhead Regional Medical Center (ARMC) is a state-of-the-art, public/nonprofit,456-bed, university affiliated teaching hospital licensed by the State of California Department of Public Health and accredited by The Joint Commission. The hospital houses a regional burn center serving four counties (San Bernardino, Riverside, Mono and Inyo), a comprehensive stroke center, level I trauma center and a freestanding in-patient behavioral health center. Additionally, ARMC operates five community-based, primary care clinics and over 40 specialty care outpatient services. CONDITIONS OF EMPLOYMENT

Pre-Employment Process: Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening.

Travel: Travel throughout the County may be required and employees will be required to make provisions for such transportation.

Basic Life Support Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.

Sponsorship: San Bernardino County is not able to consider candidates who will require Visa sponsorship at the time of application or in the future. Candidates must be able to present their legal right to work in the United States at the time of employment. 

CHW Certification: Incumbents must successfully complete a Community Health Workers (CHW) Certification program resulting in a certificate within one (1) year of hire or be terminated. 
Applicants must meet BOTH of the following EDUCATION and EXPERIENCE recruitment:
EDUCATION 
Possession of a high school diploma or equivalent is required.

-AND- 

EXPERIENCE: Applicants must meet one of the options listed below:
Option 2Certification as a Community Health Worker. A legible copy of the CHW certificate must be attached to the online application.
Option 3
One (1) year of experience as a Community Health Worker, Promotora, or Health Navigator.
Option 3: One (1) year of experience in community outreach, customer service, a medical office, human services, Behavioral Health, or a Substance Use Disorder program setting. Qualifying experience must include performing needs assessments, conducting interviews, referring clients to services, or providing crisis intervention. 




Two (2) years of experience as a Community Health Worker, Promotora, Health Navigator, or in community outreach, customer service, a medical office, Behavioral Health, or a Substance Use Disorder program is highly desirable. The ideal candidate will demonstrate cultural sensitivity and experience working with diverse and underserved populations facing barriers to wellness. Basic knowledge of medical terminology and proficiency with Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint) is also desirable.

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
