You must submit proof of immunization against influenza, pertussis, and measles from a licensed physician. You may decline the influenza vaccine. Should you decline, you will be required to complete a Statement of Declination of Influenza Vaccine, which we will provide to you upon request. .Applicants must meet minimum qualifications and submit all necessary materials in order to qualify as a candidate.
