What are the responsibilities and job description for the Triage RN position at Samaritan?

Respond to incoming calls from patients, families, and caregivers; assess needs based on symptom presentation.
Offer clinical guidance for symptom management using active listening and clinical judgment.
Triage and escalate cases requiring in-person visits, urgent interventions, or emergency services.
Communicate pertinent updates to the RN case manager, physician, and interdisciplinary team.
Coordinate urgent delivery of medications, supplies, or equipment when needed.
Notify leadership of high-risk situations or adverse events promptly.
Document appropriately in EMR all triage calls accurately and in a timely manner, including assessments, interventions, and follow-up needs.
Provide emotional support and education to patients and families navigating end-of-life.
Other duties as assigned.
