- Position Summary:
- The Transitions of Care Quality Coordinator facilitates safe, timely transitions of patients from the hospital to home by coordinating and scheduling all post-discharge follow-up appointments prior to discharge. This role serves as a clinical expert in transitional care workflows and EHR documentation, while collaborating with the multidisciplinary team to continuously improve discharge planning processes and outcomes.
- POSITION QUALIFICATIONS
- Minimum Education
- Associate Degree in Nursing (ADN)
- Minimum Experience
- Minimum of 2 years’ experience in a healthcare setting
- Required Registration/License/Certification
- Current, unrestricted Registered Nurse (RN) license in the state of practice

Schedule: Full Time, Day Shift, M-F, no call, no weekends
