In this vital role, you will support individuals in maintaining and improving their physical, emotional, and cognitive well-being through personalized care and restorative therapies. Hours: 7:30a-4p Monday through Friday
Responsibilities
- Assist residents with daily activities such as mobility, hygiene, grooming, and exercise routines to promote independence.
- Implement individualized restorative care plans under the supervision of nursing staff, focusing on improving functional abilities and quality of life.
- Monitor and document residents’ progress using electronic health records (EHR) systems. This facility utilizes Point Click Care.
- Support safe patient transfers utilizing equipment such as Hoyer lifts and adhere to safe lifting practices during manual handling tasks. Assist with training floor staff on safe life use annually.
- Lead the afternoon exercise group that has been implemented to reduce fall risks.
- Provide companionship and emotional support to residents, fostering a positive environment that encourages social engagement.
- Maintain strict adherence to HIPAA guidelines to ensure patient confidentiality and secure medical documentation practices.
- Collaborate with interdisciplinary teams—including nursing, therapy, social work, and medical staff—to coordinate comprehensive care delivery.
- Serve as a back-up to assist with transportations as needed during the day when the transportation CNA is busy.
- Assist with conducting tele-health visits between providers and residents when transportation CNA is not available.
Qualifications
- Prior experience working with geriatric patients, individuals with developmental disabilities, or in long-term care settings is highly preferred.
- Familiarity with electronic health record (EHR) systems for accurate documentation.
- Demonstrated ability to perform heavy lifting safely and utilize assistive devices like Hoyer lifts effectively.
- Strong communication skills for caregiving interactions and team collaboration.
- Experience working as RA is desired.
- Certification in CPR/BLS is required; additional certifications like dementia care or Alzheimer’s care are a plus.
