Clinical Pharmacist

Coal Country Community Health Center (CCCHC) & Sakakawea Medical Center (SMC). Full-time, primarily day shifts.

To best serve our community and the patients we care for, we are seeking a Clinical Pharmacist who is committed to excellence, growth, and patient-centered healthcare. The Clinical Pharmacist provides comprehensive pharmaceutical care to patients served by CCCHC and SMC. This role supports both outpatient and inpatient pharmacy operations, ensuring safe, effective, and evidence-based medication use. The Pharmacist works collaboratively with providers, nurses, and ancillary staff to optimize patient outcomes in a rural, team-based care environment.

This position is primarily focused on ambulatory pharmacy practice and offers a unique opportunity to help expand and enhance clinical pharmacy services across the outpatient setting. We are seeking a clinically driven pharmacist who is passionate about improving patient outcomes through direct patient care, provider collaboration, chronic disease management, medication optimization, and innovative ambulatory care initiatives. While the role is largely ambulatory in nature, the ideal candidate will also possess knowledge of inpatient pharmacy operations and be able to collaborate effectively across the continuum of care to support both community health center and hospital-based services.

  • Doctor of Pharmacy (PharmD) from an accredited program.
  • Current and unrestricted North Dakota Pharmacist License (or eligibility for licensure).
  • Clinical, ambulatory care, or hospital pharmacy experience preferred; new graduates with strong clinical training are encouraged to apply.
  • Strong communication skills and ability to work effectively in a collaborative, rural health care environment.
  • Commitment to patient-centered care, medication safety, and continuous quality improvement.

  • Support inpatient pharmacy operations, including sterile and non-sterile compounding, medication distribution, and Omnicell oversight.
  • Provide pharmacy support for infusion center and oncology services, collaborating with providers and nursing staff to ensure safe, effective, and evidence-based medication therapy.
  • Support hospice and basic care pharmacy services through medication management, interdisciplinary collaboration, consultation, and patient and caregiver education.

Patient & Provider Support

  • Serve as a medication resource for providers, nurses, and care teams across CCCHC and SMC.
  • Provide patient education on medication use, side effects, adherence, and chronic disease management.
  • Collaborate with interdisciplinary teams to support safe transitions of care and reduce medication-related hospital readmissions.

Quality, Safety & Compliance

  • Lead and support medication safety initiatives, including barcode scanning compliance, adverse drug event reporting, and policy development.
  • Participate in quality improvement projects, regulatory readiness, and accreditation activities.
  • Maintain accurate documentation and ensure compliance with ND Board of Pharmacy, CMS, HRSA 340B, and other regulatory bodies.
  • Assist with inventory management, including controlled substances, emergency kits, and formulary oversight.

340B Program Support

  • Engage in general 340B program functions, working with pharmacy and administrative staff to support program operations.
  • Collaborate with the 340B team and Third-Party Administrator (TPA) to ensure compliance, assist with audits, and maintain accurate virtual inventory processes.

Work Environment
